How do I ask a lead my own questions?
Build the questions you want on the lead you're looking at, send the link, and read the answers back on their page.
Who this is for
Any rep. There is nothing to set up first and no admin step — you build the form on the lead you're already working.
Steps
- Open the lead, find the Send request button and choose "Custom form".
- Add your questions. Each one has a type — short text, paragraph, number, date, a list to pick from, yes/no, phone or email — and can be marked required.
- Optional: make a question appear only when an earlier one was answered a certain way. Useful for "Have you been declined? → Which carrier?".
- Set how long the link should work for, or choose never expires.
- Optional: tick "Save as template" if you'll send these questions again.
- Send it. The link goes out on the lead's normal texting path, so quiet hours and DNC still apply.
A form you send is a SNAPSHOT. Editing the saved template later never changes a link already sitting in someone's texts — what they see is what you sent, which is why a form you didn't save is not a lesser one.
Where the answers go
On the lead's page, under Insurability. Insurability stays the main event; anything else you've sent sits beneath it as its own drawer, closed, with a one-line summary of where it stands. Open the drawer to read the answers. Custom answers are kept apart from the insurability worksheet on purpose — a question you invented must never count as a missing underwriting requirement.
What the lead sees
One page, your questions, nothing else. No login, nothing pre-filled, no countdown. If the link has expired, been cancelled or was already submitted, they get one plain message that doesn't say which — so a stranger with a guessed link learns nothing.
Reading the status
The drawer moves through Link created → Sent → Opened → Answered. "Link created" with nothing after it means it was built but never actually delivered.
How to check it worked
- Send one to your own phone and open it. The drawer on the lead should move to Opened within a few seconds.
- Submit it. The drawer should show Answered and list what you typed.
Common mistakes
- Treating it as the health questionnaire. Insurability is a separate form with its own rules — a custom form never feeds a quote.
- Setting a short window for something you'll chase for a week. There is no cap; "never expires" is a real choice.
- Asking for a social security number or bank details here. Those have their own rail that encrypts them — see the secure requests article.
